 UK car accident data and causes

I am trying to find a breakdown of car accident statistics on UK roads by the major causes. I particularly need to separate those accidents that are caused by driver error (which, I suspect, is most of them) from those caused by vehicle faults (eg bald tyres, faulty brakes, etc).
I have searched among the main automotive market report publishers, been to the Association of British Insurers and contacted the research centre at Thatcham. And still have almost no data.

Does anyone have any other possible sources I could check or know of any market reports that cover the subject. I'm from the auto market and am not familiar with the insurance industry, its data sources or official bodies so anyone from that background who can help would be really useful to me.
