Can anyone recommend a good OCR scanner and software to convert hardcopy text to electronic text?

It needs to be able to convert printed text, for example a contents page, and then convert it into a searchable format when the text is stored in a database. I don't mean scanners that create an image of the page. Does anyone have a successful implementation of this in practice please?
